A large housing association in Essex are recruiting for a Care & Support Assistant to join their North Essex Rapid Response Service and provide care and support to people on a short-term basis to prevent them from going into hospital or to support them when they are discharged from hospital.

The focus of this Care & Support Assistant role is to provide personal, practical and emotional support to customers living in their own homes, proving personalised care and support that enables encouraging customers to reach and maintain an optimum level of independence.
This will include the following tasks and responsibilities:
Provide personal care for the customer, assisting with personal care tasks where necessary, e.g. personal hygiene, continence management, dressing, assisting with medication.
To deliver care and support tasks as defined in the customer's assessment of need, care plan and risk assessment.
Provide assistance with domestic tasks within the customers home, e.g. laundry, bed making, cleaning, shopping and meal preparation as directed.
Provide assistance, if specified in the care plan, with managing personal finances.
Provide assistance, if specified in the care plan, with medication, within your trained level of competency.
To ensure that all records are kept up-to-date to ensure compliance
Ensure that all assistance is provided in a sensitive manner following consultation and consent from the customer and or their representative.
Record all actions / tasks undertaken at each visit on the care plan sheet and where appropriate
To report and record information, circumstances, changes or incidents concerning a client which give cause for concern
Take account of and regularly review health and safety hazards and risks within the customer's home.
To abide by the highest standards of behaviour at all times, in line with Swan Care and Support's Code of Conduct.
To participate in training as directed to ensure Swan Care and Support is delivering safe, effective and caring services.
To participate in regular supervisions in line with Swan Care and Support's policies and procedures.
To participate and contribute to staff team meetings.
To liaise with family members, friends and other professionals as appropriate.
To be considered for this Care & Support Assistant role you will the following skills and experience:
Care qualification NVQ 2 or equivalent
Liaise effectively and build positive relationships with other professionals
Excellent interpersonal skills, written and verbal
Working to deadlines and assess priorities
Working on own initiative without close supervision and as part of a team
Good attention to detail and record keeping
Knowledge and understanding of providing support, advice and the ability to communicate this knowledge.
Due to the nature of clients you will be supporting in this role, you will require an enhanced DBS.
This is a temporary contract for an initial 3 months, with a good chance of being extended or made permanent. The working hours are Monday to Friday week 1 , Wednesday to Sunday week 2, this includes a variation of hours from 7am-3pm and 3pm-11pm, 30 minutes unpaid lunch.
