Floating Support Worker

Location: St Albans/ Hertfordshire
Salary: Up to £21,569 per annum

Contract Type: Fixed Term

Weekly Hours: 35 - mon - Fri 9am - 5pm

Role purpose:

To deliver an excellent housing related support service to vulnerable adults with a mental health
condition and/ or complex needs. Customers will be based in the local community around the Watford
area.

Key results:

* Provide an effective floating support service to a caseload of customers with complex needs by
promoting independence and social inclusion and ensuring they maintain a tenancy.

* Provide specialist support with benefit applications, appeals and tribunals during one to one
support sessions and signposting to other professional services.

* Contribute to an improvement in overall health and wellbeing including but not limited to
making referrals to mental health support services/drug and alcohol services where required.

* Help reduce repeat admissions into Hospital and episodes of crisis by ensuring that all
customers receive the appropriate support according to their support needs.

* Provide support with accessing the community including attending appointments and
signposting to other relevant support agencies.

* Complete needs and assessments and support paperwork to the required standard

* Complete and report on set objectives and targets and maintain accurate records and esure all
relevant administrative tasks are completed as required.

About you:

You will have:

* Knowledge of current housing legislation & Government Policy
* Mental Health qualification / experience
* Some experience of working in housing management role
* Driving Licence and access to a vehicle
* Good time management and decision-making skills.
* Ability to work on own initiative with minimal supervision.
* Ability to visit properties and lone work as required.
* Excellent interpersonal and customer service skills.
* Excellent written & verbal communication skills.
