Hays Healthcare are actively recruiting experienced Mental Heath Nurses to work in the Community of Burton upon Trent in the Integrated Mental Health Team.

Your new role

The integrated Mental Health team in Burton on Trent are looking for an experienced mental health nurse who has a keen interest in working in a multi-disciplinary setting with patients experiencing mental health difficulties using a range approaches. You will be motivated to provide high quality care to our patients. You will need to have a drive and determination in regards to their own personal development, whilst using your own creative and innovative ideas to support the continuous development of this exciting team.

You will be responsible for undertaking clinical assessments, in accordance with the clinical pathway, in order to establish the person's needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved. They will be responsible for delivering therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

Key Responsibilities

Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
Assess patient's individual care needs, develop, implement and evaluate programmes of care.
Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process
Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
