My client is a group of luxury care homes providing care for the elderly within residential and they are looking for a care coordinator to join the team.
As Care Coordinator you will have responsibility for the effective operation and delivery of the business plan and overall care service for the unit, in conjunction with the Home Manager/DM.
You will also have shared responsibility with the Home Manager and DM for ensuring high standards of professional practice which meet the physical, emotional, social, intellectual, and spiritual needs of the residents. You will have shared responsibility with Home Manager and DM for the selection, recruitment, induction and training and regular appraisal of all staff in the unit. You will take full responsibility for the day-to-day management of the unit to the CQC and expected group standards.
You will develop, review and update the residents’ personal care plans to meet their changing physical, social and psychological needs, complete risk assessments arrange ongoing care reviews with family and/or representatives and other medical professionals.
You will promote high standards of care by ensuring all team members are trained and supervised appropriately and you will communicate professionally and warmly with visitors, family members and other health care professionals.
You must have experience within a similar role and a senior qualification within care.
