Responsible to: Team Manager
Location: Croydon
Salary: £27,882 per annum
Main Purpose of role: To prevent homelessness for substance users in treatment.
Repartees: None
Contract: Full time Monday to Friday
Key responsibilities:
• To keywork a caseload of up to 10 service users
• To fully comply with Lone working requirements.
• To ensure assessments and support plans are co-produced.
• To ensure support plans and assessments capture the voice of the service users
• To ensure the quality of support plans and assessments are reflective of our clients requirements.
• To liaise extensively with a wide range of stakeholders.
• To ensure service users have links to community supports.
• To promote good working relations and communications with stakeholders including colleagues.
• To be based in the Local Authority with co-location at the Drug and Alcohol Service
• To co-ordinate the access and options to private rented accommodation to prevent those in treatment from becoming homeless.
• To increase access to private rented accommodation and to work with landlords to encourage taking on people in drug & alcohol treatment.
• Measure the impact of increased access to the private rented sector. Work with landlords and promotion and support with service users to ensure there is continued floating housing support in a wider range of accommodations. Recruitment of floating support and bolstering existing housing schemes to provide interventions to people in recovery.
• To ensure cases are closed when required and service users fully informed.
• To travel across the borough of Croydon visiting service users when needed
• To attend staff meetings and one to ones with colleagues and team leader.
• To attend training as requested by team leader and the client
• To be proactive on issues of personal development.
• To contribute positively to the development and operation of the service through attending and contributing to team meetings.
• To fully comply with our clients policies and procedures.
