Contract: Fixed term contract until September 2024.
Hours: 28 - 35 per week.

About the role
We would love to welcome to our team an approachable and knowledgeable Dementia Adviser to work in The Luton and Dunstable hospital. This is an amazing new opportunity to provide support and guidance to people with dementia and their carers accessing the hospital and supporting discharge.
The successful applicant will work with teams in various departments throughout the hospital, including discharge planning to help people affected by dementia maintain independence and put people in more control of their own lives. We promote a person-centered service so that our support is unique to each individual, based upon their own aspirations and needs.
Your role will include:
Although this role is community based and mainly based at the hospital there will be a percentage of your week where you will work from home.
Supporting people to access vital services.
Improving people’s sense of well-being, enabling them to have more control over their lives.
Assisting with the identification of needs and provide information and access to relevant services.
Promoting our amazing service and build relationships with a range of health and social care professionals.
Supporting people affected by dementia with the transition from hospital back to ‘home’.
About you
We are looking for a highly motivated individual, with an understanding of dementia and the needs of people affected by dementia.
You will;
Be confident and able to build relationships quickly with health and social care professionals
Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
Have good communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
Have a sound knowledge of IT systems to record data, write reports and communicate by email.
Understand client confidentiality and how this is applied when representing client needs.
Preferably had some experience of working with a wide variety of relevant agencies.
Have demonstrable experience.
Have the ability and means able to travel independently.
