We have a fantastic opportunity for a locum Occupational Therapist to join a leading teaching hospital, based in South Yorkshire. They are seeking a qualified OT with previous Community experience.
Job Description
•To develop and deliver specialist Occupational Therapy for people with long term conditions and palliative care needs within the community and hospice.
•To work within the Community Therapy Service to deliver the Home First model to people in the specific area, to prevent hospital admissions and accelerate hospital discharges.
•To assess individual patient's needs, set and implement treatment plans and long term condition management for patients linking with primary, secondary and tertiary care, using a coordinated approach.
•To contribute to the development of the Occupational Therapy role within the Occupational Therapy Service by working across boundaries and interagency working.
•As part of a team, initiate research based practice and monitor the quality of the service being provided.
Position Requirements
•HCPC Registered
•Minimum of two years postgraduate experience
•Previous clinical experience performing a similar role within a Community setting
